Managing Cardiometabolic Risk

Research has demonstrated that a combination of risk factors can place an individual at heightened "cardiometabolic risk" for certain diseases, such as heart disease and diabetes. An estimated 47 million U.S residents have cardiometabolic risk which includes one out of ten children ages 12 – 19 and a 43.5% prevalence rate among adults ages 60-69.
To identify elevated cardiometabolic risk factors and reach out to persons who are most at risk for preventable health problems, a number of tools and resources, such as health risk assessments, health coaching, and incentives, are being utilized by health plans, physicians and employers. Care management programs are also focused not only on ensuring that patients with chronic conditions live longer – but also helping them live healthier lives, with fewer symptoms, so they can fully participate in the activities they enjoy. This requires a strong emphasis on preventive care, personal responsibility for healthy lifestyles, and early intervention to promote care strategies that are effective in improving the patient’s quality of life.
The seminar will highlight the results of AHIP’s survey of health plans working to identify and manage cardiometabolic health among enrollees, providers, communities, and employers. Participants will learn how health plans and employers are establishing wellness, prevention and care management programs to reduce cardiometabolic risk (or metabolic syndrome) and optimizing health outcomes.

Faculty Biographies
Beverly Everett, MD
Senior Medical Director, Chronic Condition Management
CIGNADr. Beverly Everett is Senior Medical Director with CIGNA Health Solutions and is responsible for quality management, clinical oversight and modifications, clinical effectiveness evaluations and improvements of CIGNA's disease management programs. Prior to assuming this role, Dr Everett was medical director for the Georgia Medicare Health Support pilot, which was administered by CIGNA.
Dr. Everett has been with CIGNA since 1997 and has held medical director positions with the company with responsibility for quality management, utilization management, medical cost management, network management, and appeals in Florida, North Carolina and Maryland. Prior to joining CIGNA she practiced nephrology and critical care, and later held a Clinical Instructor position in General Internal Medicine at the University of Florida. Dr. Everett is a graduate of Tufts University School of Medicine and completed her post-graduate training in both Internal Medicine and Nephrology at the University of North Carolina, Chapel Hill.
Thomas Kottke, MD, MSPH
HealthPartners & HealthPartners Research Foundation
St. Paul, MNDr. Kottke, Medical Director for Evidence-Based Health at HealthPartners, is a clinical cardiologist, epidemiologist, and health services researcher at Regions Hospital Heart Center in St. Paul and the HealthPartners Research Foundation in Minneapolis, Minnesota. He is a Professor of Medicine at the University of Minnesota. His primary research interests are improving the organization of medical practice to optimize patient outcomes and promoting lifestyle change to extend active life expectancy and delay the onset of disability.
Dr. Kottke’s publications include more than 140 peer-reviewed papers, 30 book chapters and 25 invited editorials. He was a member of both the first US Preventive Services Task Force and the federal panels that authored evidence-based guidelines for treating tobacco dependence (issued by the US Public Health Service in June 2000 and by the Agency for Health Care Policy and Research in 1996). Dr. Kottke was the principal investigator of Doctors Helping Smokers (RO1 CA 38361), the co-principal investigator of the IMPROVE project (RO1 HS 08091), and the lead author on the first meta-analysis of treatments to help people stop smoking. Dr. Kottke is on the editorial board of the Journal of Clinical Epidemiology and American Journal of Preventive Medicine.
Dr. Kottke obtained his MD degree from the University of Minnesota School of Medicine, and received his residency training in internal medicine at the Royal Victoria Hospital, McGill University (Montreal) and at North Carolina Memorial Hospital (Chapel Hill). He received an MSPH in epidemiology from the School of Public Health, University of North Carolina, Chapel Hill, and was a Robert Wood Johnson Clinical Scholar in the Department of Medicine, University of North Carolina, Chapel Hill. Dr. Kottke trained in cardiovascular diseases and preventive cardiology at the Cardiovascular Division (Department of Medicine) and Division of Epidemiology (School of Public Health), University of Minnesota.
After serving on the faculty of the University of Minnesota School of Medicine, Dr. Kottke moved to Mayo Clinic Rochester where he was a consultant in the Division of Cardiovascular Diseases and a Professor of Medicine in the Mayo Clinic College of Medicine until June 2004.
Dr. Kottke is board certified in internal medicine and cardiovascular diseases. While at Mayo Clinic Rochester, Dr. Kottke developed and directed CardioVision 2020 (http://www.cardiovision2020.org), a project to make Olmsted County, Minnesota the healthiest county in the country by the year 2020.
Michael Reardon, MD
Business & Clinical Lead, Chronic Condition Management
CIGNADr. Michael Reardon is the business and clinical lead for CIGNA HealthCare’s Chronic Condition Management Programs. His responsibilities include clinical management and oversight, program performance and profitability, vendor management, and contract negotiations. Dr. Reardon has held various leadership roles during his seven years at CIGNA HealthCare including Regional Medical Director for the Tri-State health plan.
For three years, Dr. Reardon led Aetna’s Disease Management and Woman’s Health programs. He also had responsibility for Aetna’s specialty case management and wellness initiatives.
Dr. Reardon is a Board Certified Plastic Surgeon specializing in hand surgery, repair of cleft lips and palates, facial trauma, burn care and microvascular surgery. Dr. Reardon received his medical, surgical, and specialty training at SUNY Health Science Center at Brooklyn.
Michael L. Taylor MD, FACP
Medical Director for Health Promotion
Caterpillar Inc.Michael Taylor, MD, FACP, a board certified internist, is currently the Medical Director for health promotion and disease management for Caterpillar Inc., a Fortune 50 company with revenues of $45B in 2007. Caterpillar’s wellness program, Healthy Balance®, has 100,000 participants in the US. This comprehensive program includes worksite health coaching, employee health promotion exams, biometric screening, and tobacco cessation and diabetes prevention programs.
Dr. Taylor’s previous experiences include 15 years in private medical practice and 3 years as Chief Medical Officer of Progressive Health Systems, Inc. Dr. Taylor resides in Peoria, Illinois.
