Integrating pharmacy and medical information and intelligence
into as many ongoing payer-member transactions as possible - using detailed but
easy to understand information - results in large savings and improved care, as
well as improved HEDIS and STAR measures. Precertification nurses,
care/case/disease managers, pharmacy and medical directors and others should
have instant access to understandable, actionable information related to
patients’ current medication profile.
Costly problems are identified or
prevented in a surprising number of patients. The presenters will demonstrate
an intelligent, effective systems approach that allows for such integration as
well as identifying opportunities for significant savings via proactive
outreach activities. The result is a broad-based approach to what may well be
the greatest, most expensive, and largely unaddressed problem facing the
healthcare system today: the problem of overused and misused medication.
