Care Collaboration and the Engaged Patient will address the challenges inherent in healthcare delivery and focus on the gaps in patient care that collaboration can bridge. Under the patient-centered medical home (PCMH) model, the circle of care extends beyond the physician to the care coordinator, nurse practitioner, therapist, specialist, pharmacist, and other stakeholders. To benefit from the new care delivery model, patients must be more engaged than ever before.

Part of the patient engagement formula is incenting meaningful change. Mrs. Finn and Dr. Ross will discuss strategies such as carrots versus sticks and tools that encourage the engagement process. They will examine cost incentive strategies designed to lower costs and improve the quality of care patients receive.

Mrs. Finn and Dr. Ross will highlight the necessity of advocacy. They will address the issue of who assumes the role of a patient advocate when there is no family member to fill that role. Social networking tools can play a significant part in patient engagement and sense of community. Mrs. Finn will touch on the key benefits derived from social networking communities and web resources, which can help patients navigate cost and quality measures for providers, procedures, hospitals, and other care facilities.
