The Triple Aim is intended to elevate the performance of the health care system by improving the patient experience, population health, and the cost of health care. For health insurance providers with commercial, and Medicaid populations, achieving these objectives requires familiarity with the populations. For example, 60% of Medicaid members have a chronic condition. Among the commercially insured, over 30% of members live with diabetes.
In this webinar, we’ll consider the strengths needed in a care management partner serving both commercial and Medicaid members. Learn how selecting the right partner will go far in helping a health insurance provider improve its health care quality, outcomes, financial risk.
