The Centers for Disease Control and Prevention (CDC) is targeting six chronic, preventable, and costly health conditions by using 18 proven, specific interventions. The CDC 6|18 Initiative (The Six Eighteen) is designed as a collaborative effort with payers and other stakeholders with the goal to accelerate the coverage of evidence-based preventive practices to improve the health of populations across the country.
The six targeted conditions – tobacco use, high blood pressure, healthcare-associated infections, asthma, unintended pregnancies, and diabetes – are important to health plans and other stakeholders. Many health plans have efforts underway to prevent these conditions and assist their members in better managing their health, but there is still more that can be done to help reach these important population health goals. The CDC’s scientific and public health expertise offers payers the opportunity to learn about the latest, evidence-based public health practices that can maximize outcomes associated with these efforts.

Kate Berry moderated a discussion and fielded questions on the 6|18 Initiative. This was a great opportunity for organizations to interact directly with CDC leadership on one of its key prevention initiatives. Webinar participants asked questions about how to demonstrate the impact of the prevention initiatives, working with providers, and lessons learned.
