Today’s health care system is fragmented. Providers and facilities each bill separately for their piece of treating a patient’s condition. This misaligns incentives and creates a complex system for clinicians and patients, especially those from vulnerable populations. Episodes of care are focused not on managing procedures, but on managing the patient through a whole-person approach. It captures individuals with chronic diseases who may also have many other socio-clinical risks that need to be addressed holistically. Examples of episodes include maternity, behavioral health, substance abuse as well as a range of surgical procedures. Episodes of care have shown potential to identify opportunity for better care, better outcomes, and lower cost.

This webinar will address how to leverage insights into populations of individuals with heightened socio-clinical risk to be proactive in managing referral patterns, network management, and individual care management gaps. By building an episodes of care program, we can align incentives that foster accountability, lower total cost of care, and improve patient outcomes.
