Multiple medication use among seniors in the United States has become increasingly prevalent. Research has shown that these medically complex members, who are often managing multiple chronic conditions with prescriptions across multiple prescribers, are at a higher risk for adverse drug events than their non-complex counterparts. They may also face other common barriers to proper medication use, including cost, lack of transportation, and regimen complexity. This population of seniors requires active management and a holistic understanding of their prescribed drug therapies, but they are falling through the cracks. They are in need of a pharmacy solution that works for them and with their care providers.

Tech-enabled, high-value pharmacy services are improving the pharmacy experience for medically complex Medicare members that are at risk for suboptimal medication use. This webinar will describe how targeted, analytics-backed pharmacy support has introduced a new level of patient-centered pharmacy care specifically for medically complex Medicare members. Prior to RxAnte, Chris worked at UPMC Enterprises, the venture and innovation arm of the University of Pittsburgh Medical Center, where he evaluated and recommended investments in early stage healthcare companies. Chris previously led the design and delivery of transformative, enterprise healthcare solutions for government-sponsored health plans at Deloitte Consulting.

Ms. Patel is Director of Clinical Services at Mosaic Pharmacy Service, where she oversees the development and delivery of clinical services for patients based at home and in long-term care communities. Prior to Mosaic, Ms. Patel was Director of Clinical Services for Advanced Patient Care where she provided much-needed pharmacist driven care-coordination. Combining medication delivery and continued clinical follow up resulted in increased adherence and reduced hospital readmissions.

Prior to Advanced Patient Care, Ms. Patel was Executive Director of Progress Pharmacy, another long-term care pharmacy. She led a team of 10 pharmacists and 20 technicians that served 35 assisted living facilities and 6 skilled nursing facilities. Ms. Patel holds a Bachelor of Science from University of the Sciences.
