Studies show that improving access to better nutrition, especially for those with chronic conditions, is an effective way to reduce medical costs and improve overall health. Especially in light of recent legislative changes to Special Supplemental Benefits for the Chronically Ill in Medicare Advantage plans, health plans have a unique opportunity to design programs that can result in better condition management.

Persons with chronic health conditions often do not have access to, or use of, nutritional support that can help improve their health while managing disease or resulting disability. Thus, it is more difficult to control other key behaviors including medication adherence, and often results in lowered health status that then can lead to increased reliance upon/use of high-cost health care resources.

Attendees will learn how home delivery of nutritionally-focused meals can positively affect the management of chronic health conditions and lower utilization costs.
