People love to eat healthy food. In a survey of more than 1,000 Medicare beneficiaries with a chronic condition, eating healthy meals was the top-rated self-care activity. Medicare Advantage plans and Medicaid MCOs are adding home-delivered meals programs for members after discharge from an inpatient stay and for the management of chronic conditions. The vast majority – 81 percent — of those 65 and older have two or more chronic conditions. And people with two or more chronic conditions are more likely to have unmet social needs. An environmental scan of 2018 health care trends from leading organizations uncovers that a top trend is incorporating social determinants of health into care management models. Medicare Advantage plans can attract, support and engage members by including condition-appropriate home-delivered meals as a supplemental benefit.
