Coordinating the post-acute care services patients need to heal or age at home can be quite complex. Finding the right in-home care team, and then ensuring their services are covered benefits, can be especially complicated. Overcoming these challenges requires a multi-faceted management approach. This session will explore benefit designs that promote a “home-first” approach, and policy changes and reimbursement strategies that support care at home. Join us to learn best practices in coordinating care between in-patient settings and the home.
