Chronic diseases are among the most prevalent and costly health conditions in the United States. Nearly half of all Americans suffer from at least one chronic disease, and that number is growing. According to the Centers for Disease Control (CDC), one in four adult patients have multiple chronic diseases and that number jumps to 3/4 of all individuals over the age of 65. In the U.S. alone, chronic diseases account for nearly 75 percent of all healthcare spending and are responsible for 7 out of 10 deaths. Beyond the skyrocketing cost of care, managing multiple chronic conditions poses complex challenges for individuals who are juggling treatments, fighting fatigue, facing physical limitations and who are hamstrung by emotional distress. And, although individuals with chronic conditions know they must make self-care a priority, according to a survey from the American Heart Association, less than 50% do.
