The total costs of health care in the U.S. for chronic health conditions is more than $1 trillion annually. Costs are expected to continue to rise, and there are many solutions available to help health insurance providers, employers and health systems manage member health and lower costs. So many solutions — it’s overwhelming! Payer challenges include keeping up with all the options, figuring out which is most cost-effective, and getting people to engage. Additionally, with each added vendor, there is another layer of complexity for the administrator and the end-user to navigate. Both health insurance providers and health care consumers want comprehensive, personalized, and thoughtfully-integrated solutions.
Often, an individual is diagnosed with more than one chronic disease. Wouldn’t it be great to have a single coaching platform to support comorbidities and address social determinants of health, rather than having multiple vendors in the mix? Imagine the simplified member experience and your reduction in vendor fees! During the webinar, Pack Health will share our comprehensive solution for chronic condition management. We serve members with autoimmune conditions, cancer, chronic pain, obesity, diabetes and prediabetes, heart disease, hypertension, pulmonary conditions, chronic kidney disease, depression, and more.
We’ll provide detailed information surrounding our effective engagement strategy, tips for engagement that cuts across populations, how we address barrier and access issues, and the ways in which we leverage device integrations such as wearables, weight scales, and glucose meters. We’ll also discuss Pack Health’s flexible payment options and why we believe a personalized, member-focused approach to chronic disease management is more impactful and efficient than a one-size-fits-all approach to general wellness.
By the end of the webinar, attendees will learn and understand:
