Today, healthcare organizations recognize the impact of a patient’s social situation on his or her health status, and there is evidence that addressing a patient’s social needs has a positive downstream effect on that patient’s health. These organizations are challenged to identify and deliver services and interventions that target these social determinants of health (SDOH).
This white paper explores how evidence-based practice can help organizations provide quality, cost-effective care, thereby mitigating SDOH and improving patients’ lives.
