National Vice President of Strategy, Complex Care Management,
Diane Flynn the National VP of Strategy for Optum Care, Complex Care Management (CCM) and has been with Optum for 17 years. Her past roles include Regional President for CCM Central Region, and VP of both the Florida and New York City Institutional Special Needs Plan (ISNP). The CCM Institutional program (formerly Evercare) offers a special Advanced Practice Clinician (APC) clinical model of care to Nursing Home residents enrolled in an ISNP, MMP, or MAPD health plan and an APC model of care to manage length of stay and prevention of readmissions for short stay residents of the SNF.
Prior to joining Evercare, Diane was the VP and Chief Nursing Officer for Lakewood Hospital, in Cleveland Ohio, part of the Western Region of the Cleveland Clinic System. She also spent her earlier career years as a Director of Surgical Services in several hospitals in Illinois and Florida. Diane has an M.S. degree in Nursing from Northern Illinois University and a B.S.N. from Marycrest College in Davenport, Iowa.
