Eric Coleman, MD, MPH

Dr. Eric Coleman is an international leader in geriatric and chronic disease care whose work focuses on the many challenges that occur as patients transition from hospitals to post-acute care and to home. Dr. Coleman was recognized with a MacArthur Fellowship “Genius Grant” for his work in bridging innovation and practice through enhancing the role of patients and family caregivers in improving the quality of their care transitions. Dr. Coleman serves as the Director of the National Care Transitions Program (www.caretransitions.org).

Dr. Coleman received a Bachelor’s of Science degree in Public Health (1987) from the University of California at Davis, a Master’s in Public Health degree (1991) from the University of California at Berkeley, and a Doctor of Medicine (MD) degree (1992) from the University of California at San Francisco. He completed his residency (1993–1995) in primary care internal medicine and fellowships in health policy and geriatric medicine (1995–1998) at the University of Washington in Seattle.
