June Simmons, MSW, has enjoyed a long career as a health care executive in hospital and home health settings and founded Partners in Care in 1997. June has pioneered development and scaling of evidence-based innovative interventions for management of medications at home, self-management of chronic conditions, coordinated care to improve health outcomes and home palliative care.
Throughout her distinguished career, she has been instrumental in envisioning, creating, funding, and operating forward-looking health and social service programs, which meet the mutual needs of patient populations, health care delivery networks and health plans. Her priority is sustainable patient-driven integration of care from hospital to home and community in the 21st century.
Partners continues to serve as a catalyst for shaping a new vision of health care by partnering with organizations, families, and community leaders in the work of changing health care systems, communities and lives. They are building prototype regional community delivery systems for the new health environment, and work to scale through shared learning systems at the national level.
