Home Delivered Meals and Disease Management: A Powerful Combination to Control Diabetes

About This Webinar

More than 12 percent of the adult population in Nevada has been diagnosed with diabetes and many do not know they have it. Diabetes and its complications disproportionately impact native American, black and Hispanic populations. Eating regular, balanced meals and snacks each day is critical for the management of diabetes; however, this is a particular challenge for beneficiaries who may suffer from food insecurity.

The Health Plan of Nevada developed an innovative approach to diabetes management that integrates home-delivered meals into its disease management program to help members receive the education, motivation and support they need to learn how to best self-manage their diabetes. Members speak with a nurse over 4 weeks prior to receiving meals, and then maintain weekly phone calls while receiving 8 weeks of meals.

Data from the 12-week program demonstrate improved hemoglobin A1c levels and medication adherence, as well as decreased use of overall healthcare spend. By adding meals to its disease management program, the health plan is moving to a more holistic model of care that addresses a combination of medical and social needs to improve lives and deliver cost savings.

Through a presentation of the program protocol and analysis, attendees will gain an understanding of how incorporating home-delivered meals into a disease management program can lead to improved clinical outcomes and medication adherence as well as reduced total cost of care in members with diabetes.
