About this Webinar
Members with high utilization are more likely to report unmet social needs. A managed care plan developed an innovative pilot to support high-risk members with a behavioral health diagnosis with 12 weeks of home-delivered meals. Presenters will discuss how meeting this basic need contributed to positive outcomes for individuals and the health plan, impacting provider and case manager visits, in-patient stays, readmissions and total cost of care. Attendees will gain insights from lessons learned.
Attendees Will Be Able To
- Identify a population that may benefit from a nutrition intervention
- Describe the outcomes of a pilot program providing home-delivered meals to vulnerable members with a behavioral health diagnosis
- Understand the pathways for financial support of such a program in Managed Medicaid and Medicare Advantage programs
