A study reported in March 2022 in the American Journal of Managed Care found that the 40% of Medicare beneficiaries designated as having highly fragmented care had a mean of 13 ambulatory visits across 7 practitioners in the year. The most frequent provider of care accounted for only 28% of visits. Using Medicare beneficiaries as our example population, if delivery of coordinated high-quality, low-cost care for vulnerable member populations is our north star, we clearly have opportunity for improvement. Primary care has a significant role to play here. And we can learn from successful, tried and true models deployed in U.S. safety net care systems and in global settings.

During this session, experienced presenters will share their experiences and views on a reimagined approach to care for vulnerable members and how we can leverage models which have proven themselves to be highly successful, even in resource-constrained settings.
