Less than 16% of eligible patients in the U.S. participate in cardiac rehab due to barriers in access, thereby driving up cardiac hospital readmissions. Virtual cardiac rehab can improve access to care using a virtual care model that improves outcomes and health equity while reducing cardiovascular cost of care.

This webinar will address these issues by explaining how virtual care and value-based payment models can be leveraged to improve patient engagement and reduce hospital readmissions and total cardiovascular cost of care. This virtual care delivery maintains the same safety and efficacy as that of traditional, in-person cardiac rehab programs.
