About This Webinar

Providing time-sensitive care for instances tracked by HEDIS is critical, particularly for high-risk members at vulnerable points in their care journey. This session explores how Healthfirst, a 4.5 Star provider-sponsored plan, collaborated with InterSystems and its network providers to leverage near real-time visit data for automating the identification of qualifying events for time-sensitive HEDIS measures and alerting treating providers. Attendees will learn how interoperability enables timely care and improves quality.

This session will provide a recent case study of how a health plan leveraged advanced interoperability to achieve double-digit improvements on adherence rates for time-sensitive HEDIS measures. The case shows how plans can use clinical data streams now widely available to proactively trigger needed care in tight windows for high-risk members.

Attendees Will Learn About

  • How to leverage near-real time clinical data to improve performance on time-sensitive HEDIS measures
  • How to trigger timely follow-up care for at risk members within narrow time windows
  • How to apply automation to optimize care coordination and quality outcomes
