Too often, payers and health care providers are at odds when it comes to treating patients, especially vulnerable and complex populations. The speakers will share tools for working together to identify, engage and provide care for Medicaid members, including a combination of a core, integrated team with a community outreach team. This model has demonstrated significant outcomes in reducing unnecessary hospital visits and lowering health care costs while increasing consistent primary care visits and instilling trust in patients.
