We’ll also examine how tackling these barriers to care can improve outcomes, lower care and utilization costs, and deliver a stronger return on investment for health insurance plans.
We've found that 60% of the members we engage have at least one unmet social need — with an average of 3.4 per member. While specific needs may vary by Medicaid, Medicare, or commercial plans, the underlying barriers remain widespread. Through our national network of community-based organizations (CBOs) and hyperlocal, culturally-tailored, human-to-human outreach, we’ve successfully connected with hard-to-reach populations, improving member engagement, reducing unnecessary utilization, and driving cost avoidance. During this session, we’ll share actionable approaches and real-world outcomes that health insurance plans can implement to boost engagement and ROI by solving these barriers to care.
